29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap a) New residential construction, Lot 11, Exterior floor: On or about 4/28/2021, and at times prior, the employer exposed employees performing framing work to shock in that the extension cord that was being used to operate portable hand tools was spliced into another smaller size cord with masking tape.

29 CFR 1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws a) New residential construction, Lot 11, Exterior floor: On or about 4/28/2021, and at times prior, the employer exposed employees conducting framing activities to shock hazards, in that the strain relief on the extension cord being used to power a portable hand tool was damaged and pulled away from the terminal screws.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) New residential construction, Lot 11, Roof: On or about 4/28/2021, and at times prior, the employer exposed an employee installing sheathing to fall hazards, in that a complete fall protection system was not provided when he was working on an 8:12 pitched roof at heights measured at 22 feet 6 inches to the lower level.
