An orderly shutdown must be utilized to avoid any additional or increased hazards to employees as a result of the equipment stoppage.  On or about 5/04/2021, Daddy Ray's Inc. had not ensured equipment was locked out prior to servicing the equipment.  A conveyor in line # 2 was accumulating food product causing the belt stall or an equipment stoppage.  Employees were grabbing and pulling the belt to restart the conveyor without locking-out the equipment with the applicable lockout procedure.

The guarding device was not in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, was so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about 5/04/2021, employees at Daddy Ray's Inc. were exposed to point of operation hazards.  The following equipment did not have guards:  a)	Monarch Lathe, Model #612/Serial # 44236, was missing chuck and chip guards; b)	Bridgeport Milling Machine in maintenance area was  missing a chuck guard; c)	Wilton Drill Press, Model # 2500/ Serial # 00319, was missing a chuck guard.
