29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  New residential construction located on Lot #186 at 39 Cherry Tree Bend, Newnan, GA 30265, Second level top plates: On or about 04/27/2021, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that employees were not provided with protective eyewear while engaged in roof truss installation operating these tools at 120 PSI from the air compressor.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    New residential construction located on Lot #186 at 39 Cherry Tree Bend, Newnan, GA 30265, Second level top plates: On or about 04/27/2021, the employer exposed employees to fall hazards of 19-feet, 4-inches to the lower ground level, in that employees were not attached to a life-line or lanyard while wearing full body harnesses installing roof trusses without a connection to the back D-Ring.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)    New residential construction located on Lot #186 at 39 Cherry Tree Bend, Newnan, GA 30265, Stairwell located on first level: On or about 04/27/2021, the employer exposed employees to fall hazards of 10-feet, 4-inches to the lower concrete slab, in that the ladder used to access the upper level was not extended at least 3-feet past the landing or secured to prevent from moving during use.
