29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 05/04/2021, at the job site - employees were exposed to a 24 foot 6 inch fall hazard from a 4:12 pitched roof while conducting re-roofing work, in that, fall protection was not used.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about 05/04/2021, at the job site - the rails of the two ladders used to access the roofs of a three story residential building did not extend at least 3 feet above the upper landing surfaces, exposing the employees to 24 feet 6 inch and a 14 feet 3 inch fall hazards.

On or about 05/04/2021, at the job site - employee were not trained to recognize hazards related to ladders, in that, employees were using ladders where the rails did not extend at least 3 feet above the landing surfaces, and an employee was climbing ladders while holding a box of nails over his shoulder exposing the employees to 24 feet 6 inch and a 14 feet 3 inch fall hazards.
