29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 3, 2021, and May 5, 2021, an employee was exposed to amputation hazards due to the lack of adequate machine guarding on the Rod Excello in the Machinery Department, in order to prevent employee exposure to an amputation hazard created by the clamp securing the product.
