On or about May 5, 2021, the employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix). The employer required employees to wear N95 Inovel respirators while exposed to suspected or confirmed positive COVID-19 residents. Employees provided care to residents, provide meals, housekeeping, maintenance, and COVID-19 tests on residents suspected to have COVID-19.
