29 CFR  1926.451(b)(5)(ii):Ends of platforms greater than 10 feet in length extended more than 18 inches over the centerline of its support, without being designed and installed so to prevent tipping of the cantilevered end loaded with employees and/or materials or provided with guardrails to prevent employees from accessing the cantilevered end.  Location - 1651 Main Street, Concord, MA.     On or about May 6, 2021, employee's were working on a carpenters bracket scaffold platform that had greater than eighteen inches of overhang on both ends.

Location - 1651 Main Street, Concord, MA.  On or about May 6, 2021, employees were working/walking on first and second floor decking of a residential house that had unprotected floor openings.

Location -  1651 Main Street, Concord, MA.  a) On or about May 6, 2021,  employees were working on a roof not connected to fall protection system at a height greater than fourteen feet above lower level.  b) On or about May 6, 2021, Employees were exposed to fall hazards from unguarded window openings on the first floor.
