29 CFR 1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: Employees working in the main grind room were exposed to amputation hazards, in that the sprockets on the combo dump were not guarded. The two sprockets, one located within approximately 1/4 inch of the handrail for the stairway leading to the top of the Butcher Boy, and 55 inches from the sprocket center to the concrete floor, and the second located at the combo dump control station were not enclosed.
