29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing operations.  This was most recently observed on May 6, 2021 at 4704 N 186th St, Elkhorn, Nebraska when 4 workers were installing roofing materials to a residential structure with an eave height of 12 feet above the ground and they were not using fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders during residential roofing activities.  This was most recently observed on May 6, 2021 at 4704 N 186th St, Elkhorn, Nebraska when 4 workers were observed using a ladder to ascend the roof of a residential structure and the ladder's side rails did not extend at least 3 feet past the upper landing surface.
