On or about May 7, 2021, at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, thirteen (13) feet two (2) inches above the ground, did not have the working level fully planked, thereby exposing the employee to fall hazards. b. On or about May 7, 2021, at the above addressed jobsite, an employee moving material from the first tier to the second tier of a tubular welded frame scaffold, six (6) feet six (6) inches above the ground, did not have the working level fully planked, thereby exposing the employee to fall hazards.

Crossbraces were used as a means of access. a. On or about May 7, 2021, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold were not provided with an access ladder, thereby exposing the employees to fall hazards and struck by hazards.
