On or about May 7, 2021, at the construction site located at 3680 Quarter Pole Lane, Grove City, Ohio, the employer failed to maintain a safety and health program to ensure hazard detection and correction. Pedro Sierra was previously cited for a Serious violation of this Occupational Safety and Health standard 1926.20(b)(1), which was contained in OSHA inspection number 1330897, citation number 1, item number 1 and was affirmed as a final order on November 23, 2018, with respect to a workplace located at 110-116 Pioneer Circle, Pickerington, OH 43147.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about May 7, 2021, at the construction site located at 3680 Quarter Pole Lane, Grove City, Ohio, employees were performing roofing work on a one story condominium building with a pitch of approximately 8 in 12 and approximately 10 feet in height without fall protection, thereby exposing the employees to a fall hazard. Pedro Sierra was previously cited for a Serious violation of this Occupational Safety and Health standard 1926.501(b)(13), which was contained in OSHA inspection number 1330897, citation number 1, item number 3 and was affirmed as a final order on November 23, 2018, with respect to a workplace located at 110-116 Pioneer Circle, Pickerington, OH 43147.
