29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures a) BPA Unit PSM covered process area: On or about, May 3, 2021, and at times prior to, the employer failed to verify that BPA Formulation Pump L3P1090 had been isolated from all active energy systems prior to permitting employees to perform a leak test upon BPA Formulation Pump L3P1090 exposing employees to chemical burn hazards.
