On or about May 10, 2021, at the above addressed jobsite, employees using a pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about May 10, 2021, at the above addressed jobsite, an employee performing roofing operations on a roof eighteen (18') one (1") inch above the ground was not protected from falls, thereby exposing the employee to fall hazards.
