29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  The employer is failing to protect employees from fall hazards associated with inadequate inspections leading to possible scaffold collapse. This was most recently documented on April 21, 2021, at 13800 Metcalf Overland Park, KS.  The employer had employee(s) working on a scaffold system that had not been inspected by a competent person prior to use thus failing to identify that planks were too long for the length of this scaffold frame.

29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration: Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  The employer is failing to protect employees from fall and tripping hazards associated with inadequate erected, moved, dismantled, or altered scaffolds.  This was most recently documented on April 21, 2021, at 13800 Metcalf Overland Park,  KS. The employer allowed an employee(s) to erect and use a scaffold without the supervision and direction of a competent person.  The employee(s) erected the scaffold using unsupported planks that were too long for the length of the scaffold frame leaving an unsupported cantilevered end.
