29 CFR 1910.134(c)(2)(ii):The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user. On or about May 10, 2021 employees who are required to wear a respirator were not medically evaluated before they were allowed to use the respirator. This condition exposed employees to health hazards.

29 CFR 1910.1030(f)(2)(i):Hepatitis B vaccination was not made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. On or about May 10, 2021, employees who clean human bodily fluids and other infections materials were not offered the hepatitis B vaccination within 10 working days of their initial assignment. This condition exposed employees to infectious diseases such as contracting hepatitis A or B and other viral infections..

29 CFR 1910.1030(g)(2)(i): The employer did not train each employee with occupational exposure in accordance with the requirements of this section. On or about May 10, 2021, employees with occupational exposure to human bodily fluids and other infectious materials were not trained in regulatory content, epidemiology, symptoms of blood borne diseases, modes of transmission, etc., as required by the standard. This condition exposed employees to health hazards such as but not limited to HIV, Hepatitis B, etc..
