29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Residential and commercial client sites - on or about April 23, 2021, the employer failed to develop and implement a hazardous energy control program (lockout/tagout) in the workplace. Employees engaged in maintenance, servicing, and installation tasks on garage door systems were exposed to struck-by hazards likely to result in serious injury upon unexpected startup. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. On or about May 11, 2021, the employer did not keep OSHA Form 300 "Log of Work-Related Injuries and Illnesses" for year 2021 and did not complete OSHA Form 301 "Injury and Illness Incident Report" for April 23, 2021 recordable injury resulting in days away from work. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
