29 CFR 1910.140(c)(10)(i):(i) Snap hooks and carabiners must not be connected to any of the following unless they are designed for such connections - Directly to webbing, rope, or wire rope: On or about May 11, 2021 employees accessing the top of the PHL Unscrambler on the Green Line were wrapping the lanyard around the steel tube that ran across the machine and connecting the lanyard's snap hook onto the lanyard's webbing to be used as an anchor, exposing employees to a fall hazard due to a potential failure of the snap hook and/or lanyard from improper application of use.
