29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  On or about May 11, 2021 an employee was working from an aerial lift without a harness with lanyard attached to the boom or basket. This condition exposed the employee to a fall hazard greater than 6 feet from the ground.
