29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 6, 2021 the employer failed to ensure that effective machine guarding was in place on the Hannifin Dyna-Quip hydraulic press (model OGF-8b, serial number 50171). The press was actuated by foot control without other effective measures in place to protect the employee from reaching into the hazardous area. Employees were not protected from the point of operation where the press action takes place or the closing action of the dies themselves. T & D Fabricating was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212 (a)(1), which was contained in OSHA inspection number 1396979, citation number 1, item number 6 and was affirmed as a final order on October 2, 2019 with respect to a workplace located at 1489 E. 363rd St., Eastlake, OH 44095.
