On presses with provision for manual lubrication of flywheel or bullgear bearings, lubrication was not provided according to the press manufacturer's recommendations;  a.)	On or about May 7, 2021, the employer did not ensure employees were protected from crush hazards while operating the #84 60T Rousselle part revolution mechanical power press (Serial# 23234). An employees sustained a finger amputation after the ram fell while pulling a part after a bearing failed due to inadequate greasing.
