A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. a) On or about May 12, 2021, the exit door from the baking area had a broken screen door in front of the exit door that was hinged on the inside and was required to be pulled inwards prior to opening the exit door.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about May 12, 2021, the employer had not provided guarding on the Hobart mixing machines for machines such as but not limited to the Hobart Floor Mixer Model M-802 SN 1315307 and the Hobart Table Mixer tagged as Forest Hills School District T9002238. Employees were exposed to point of operation hazards from the mixing paddles while working on or near the machines.
