Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage:  8115 Break Away Pass, front side of the building: On or about April 21, 2021, and at times prior thereto, employees were exposed to a fall hazard greater than 10 feet to the ground below while installing weather barrier from a pump jack scaffold that did not have metal brackets.
