The forklift and attachment were used to move materials throughout the warehouse.  b) Warehouse, various interior: On or about 5/11/2021, and at times prior, the employer exposed employees to physical hazards, in that they were using a Cascade Turnaload attachment (SER# 26J-TAL-3A-0230) on a Clark electric lift (Model# ECX30, SER# ECX360-0465-9978FL) without the prior approval of the manufacturer. The forklift and attachment were used to move materials throughout the warehouse.
