29 CFR 1910.178(a)(4):Capacity, operation, and maintenance insturction plates, tags or decals of the powered industrial truck were not changed accordingly a) Warehouse, various interior: On or about 5/11/2021, and at times prior, the employer exposed employees to physical hazards, in that they were using a Long Reach Clamp attachment (LCC35NSB4848STD0AQ) on a Clark electric lift (Model# ECX25, SER# EECX360-1109-9653KE) without the prior approval of the manufacturer. The forklift and attachment were used to move materials throughout the warehouse. b) Warehouse, various interior: On or about 5/11/2021, and at times prior, the employer exposed employees to physical hazards, in that they were using a Cascade Turnaload attachment (SER# 26J-TAL-3A-0230) on a Clark electric lift (Model# ECX30, SER# ECX360-0465-9978FL) without the prior approval of the manufacturer. The forklift and attachment were used to move materials throughout the warehouse.
