29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about May 12, 2021, on the north side of the construction site, employees were performing roofing work without proper fall protection at heights upwards of fifteen (15) feet.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: on or about May 12, 2021, on the north side of the construction site, employees were utilizing a Werner twenty (20) foot extension ladder to access work area, ladder was not extended three (3) feet above landing.
