29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to: Crushing hazards as employees were operating a powered industrial truck in a construction site and were not wearing a seat belt.   a.	At the construction site, located at 17210 SR 438, Oakland, FL , an employee was exposed to struck-by/crushing hazards, when he was observed operating a GENIE Forklift Model GTH1056 S/N GTH1016E-11363 without a seat belt. He was delivering materials within the site.  b.	At the construction site, located at 17210 SR 438, Oakland, FL ,  an employee was exposed to struck-by and crushing hazards, when he was observed operating a GENIE Forklift Model GTH1056 S/N GTH1014-18925 without a seat belt. He was offloading trash from the upper building floors.
