29 CFR 1910.178(l)(2)(i): Trainees may operate a powered industrial truck only: (A) Under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence; and (B) Where such operation does not endanger the trainee or other employees. Location: 3580 East Main Street, Waterbury, CT 06705 - On or about May 2, 2021, a trainee was allowed to operate a powered industrial truck under the supervision of another employee who did not have the knowledge, training, and/or experience to train operators and evaluate their effectiveness. As a result, the trainee accidentally struck another employee who incurred serious injuries, including a broken ankle and a ruptured liver.
