29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, and flange projections.     (a) On or about May 13, 2021, Partners Manufacturing Inc. did not ensure that the Okamoto 6-12/14 surface grinder was guarded to cover the spindle end, nut, and flange projections.  (b) On or about May 13, 2021, Partners Manufacturing Inc. did not ensure that the Ryobi 8" bench grinder was guarded to cover the the spindle end, nut, and flange projections.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with you certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support off-hand grinding work and/or work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth inch:   (a) On or about May 13, 2021, Partners Manufacturing Inc. did not ensure that work rests to support grinding work, were installed  and adjusted on the Ryobi 8" bench grinder,  located in the production area.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with you certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

(a) On or about May 13, 2021, Partners Manufacturing Inc., did not ensure that tongue guards were installed and adjusted to one-fourth inch to support grinding work, on the Ryobi 8" bench grinder located in the production area.  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification.
