The employees are permitted to walk where there is water on workplace floor. On 5/13/2021 employees were observed walking on wet floors in non-production area.

29 CFR 1910.28(b)(7): The employer did not ensure that each employee on a walking-working surface near an opening 4 feet (1.2 m) or more above a lower level is protected from falling:     5050 Kemper Ave, St. Louis MO 63139: Employer is exposing employees to hazardous conditions where the employees are permitted to use a stairway which have unguarded openings.  On and about 5/13/2021 employees used ship stairs above which there was an opening greater than 48 inches to the floor.  The employees were exposed to fall hazards.

On 5/13/2021 employees were observed using a door for exit purposes.  The door did not have a sign identifying it as an exit door.

On 5/13/2021 an employee operated a forklift which did not undergo required safety examination to identify defects such as, but not limited to, non-functioning hazard warning light thereby exposing employees to struck-by hazards.

On 5/13/2021 employees were exposed to amputation hazards where the guard openings on floor fans were larger than 1/2 inch.  The blade peripheries of the fans were less than 4 feet above the workplace floor.
