29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about May 11, 2021, employees were exposed to struck-by hazards during siding tear-off activities when the employer did not ensure a hard hats were utilized. No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. On or about May 11, 2021, employees conducting siding installation operations from a pump jack scaffold at a residential single-family property were exposed to fall hazards of approximately 11.5' when the employer failed to ensure that fall protection was utilized. No abatement certification or documentation is required of this item.
