29 CFR  1926.451(b)(6): On scaffolds where scaffold planks were abutted to create a long platform, each abutted end did not rest on a separate support surface:  a) Worksite, 161-31 91st St., Howard Beach, New York 11414:  On or about May 13, 2021, employees were performing exterior siding  work from a platform on a pump-jack scaffold approximately 14-feet above the ground. The work platform did not rest on a separate support surface exposing employees to fall hazards.  Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  a) Worksite, 161-31 91st St., Howard Beach, New York 11414, North Side of Building:  On or about May 13, 2021, employees were engaged in installation of siding on a pump-jack scaffold that was constructed less than three feet from an energized power line that was at 300 volts or less, exposing the employees to electrical hazards.   b) Worksite, 161-31 91st St., Howard Beach, New York 11414, West Side of Building:  On or about May 13, 2021, an employee was engaged in installation of siding on a pump-jack scaffold that was constructed less than three feet from an energized power line that was at 300 volts or less, exposing the employees to electrical hazards.   Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.

The pump-jack scaffold poles were not secure at the bottom to the residential building exposing employees to fall hazards.  b) Worksite: Worksite, 161-31 91st St., Howard Beach, New York 11414 West Side of Building:   On or about May 13, 2021, an employee was performing exterior siding work from a platform on a pump-jack scaffold approximately 16-feet above the ground. The pump-jack scaffold poles was not secure at the bottom to the residential building exposing the employee to fall hazards.  Note: In addition to abatement certification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
