29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about May 13, 2021, 5 employees installing roofing material with an air driven air nail gun were exposed to potential eye injury, in that no eye protection was worn during the process.     The DCD Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Eye and Face Protection), which was contained in OSHA inspection number 1449949, citation number 01, item number 01 and was affirmed as a final order on 3/17/2020, with respect to a workplace located at 159 Lake Ave NE, Massillon, Ohio, 44646.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a) On or about May 13, 2021 employees were working on the 12:12 pitched roof at a height of 24.5 feet of a single family home without the use of PFAS or protected by any other form of fall protection system being used.    The DCD Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have Protection), which was contained in OSHA inspection number 1449949, citation number 01, item number 02 and was affirmed as a final order on 3/17/2020, with respect to a workplace located at 159 Lake Ave NE, Massillon, Ohio, 44646.  The DCD Construction Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have Protection), which was contained in OSHA inspection number 1475976, citation number 01, item number 01 and was affirmed as a final order on 8/16/2020, with respect to a workplace located at 1519 Walnut Rd SE, Massillon, Ohio, 44646.
