29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers:   (a)  South Outside Form Wall - On or about May 5, 2021 the employer did not ensure employees were protected from falls when working above ten feet from scaffolding, exposing employees to fall hazards.

29 CFR  1926.651(i)(3):Sidewalks, pavements and appurtenant structure shall not be undermined unless a support system or another method of protection is provided to protect employees from the possible collapse of such structures.  (a) North Jobsite - On or about May 5, 2021 the employer did not install a support system for an undermined concrete sidewalk exposing employees to a struck-by hazard.

29 CFR  1926.651(j)(1): Adequate protection shall be provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face. Such protection shall consist of scaling to remove loose material; installation of protective barricades at intervals as necessary on the face to stop and contain falling material; or other means that provide equivalent protection.    (a) North Jobsite - On or about May 5, 2021 the employer did not install a support system for a vertical wall excavation exposing employees to a struck-by hazard.
