29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Worksite located at 210 W. Truman Road, Independence, Missouri- Employees were exposed to a fall hazard in that employees were working on a fabricated framed scaffolding and were not protected from falling by the use of a guardrail system.

29 CFR 1926.1053(a)(8):A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used: Worksite located at 210 W. Truman Road, Independence, Missouri- Employees were exposed to fall hazards in that the stepladder was being misused in that the metal spreader bar was locked into place while the employees were using the ladder to access the scaffolding.
