29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Y&J Construction LLC at 121 1st Avenue Southeast in Watertown, SD 57201: On or about and at times prior to May 12, 2021, the employer did not ensure employees were protected from fall hazards when installing sheathing in that one employee straddled the hip of the roof without utilizing any fall protection system. This condition exposed one employee to fall hazards of approximately 20 feet.   (b) Y&J Construction LLC at 121 1st Avenue Southeast in Watertown, SD 57201: On or about and at times prior to May 12, 2021, the employer did not ensure employees were protected from fall hazards when installing sheathing in that two employees utilized one reusable roof anchors, butterfly anchor, that was secured to the roof using 6 screws. The plates of the reusable anchor were not adhered to the surface of the roof. This condition exposed employees to of approximately 20 feet. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) Y&J Construction, LLC at 121 1st Avenue Southeast in Watertown, SD 57201: On or about and at times prior to May 12, 2021, the employer did not ensure employees were protected from fall hazards when transitioning from the ladder to the roof in that the ladder was not extended at least three feet above the roof eave. This condition exposed employees to fall hazards of approximately 20 feet.
