29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May, 13, 2021, the employer did not ensure that employees of a subcontractor, performing roof work activities, were in use of fall protection. This exposed the employees of the subcontractor to a fall hazard greater than six (6) feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about May 13, 2021 the employer did not ensure the self-supported A-frame ladder was used only in the open position. The employees were in use of the ladder in the closed position leaned against the roof and this exposed the employees of a subcontractor using the ladder to a fall hazard.
