29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 05/11/2021 at the loading/unloading bridge area; where an employee was performing servicing/maintenance activities (clearing a jam on the TAK Unloader) without deenergizing and locking the energy isolating device (disconnect switch) in the "off" position. The employee was exposed to the shearing/crushing hazard created by the unloading arm which started automatically. ABATEMENT DOCUMENTATION REQUIRED
