29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) At the site, on or about 5/17/2021, employees working on a mezzanine with an unprotected edge of 9 ft-6 in above the lower level were not protected from falling by an adequate guardrail system, safety net, or personal fall protection system.

29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface.   a) At the site, on or about 5/17/2021, the employer did not ensure that employees working on a mezzanine with an unprotected edge that was 9 ft-6 in above the lower level was protected by a guardrail system, as the wooden guardrail system used did not have a midrail installed.

29 CFR  1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  At the site, on or about 5/17/2021, a light switch in the hallway between the office and production area, two junction boxes in the tool room, and 22 junction boxes in the production area were missing covers.
