29 CFR  1910.178(f)(2):29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel shall be in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in Sec. 1910.6:  (a) Powered Industrial Vehicles (PIV): The employer was utilizing propane cylinders which were past their hydrostatic test date, exposing employees to struck by hazards.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)Throughout the facility; The employer did not develop an Energy Control Program for employees required to perform maintenance, repairs, cleaning, and tool changes for equipment such as but not limited to band saws and CNC Lathes at the facility, exposing employees to amputation hazards.
