29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a.  Truss Manufacturing Plant: On or about 5/17/2021, the employer exposed employees to caught-in and amputation hazards, in that a lockout tagout program was not developed with specific energy control procedures for the Alpine AutoMill RS, among other equipment at the site with potential to release stored hazardous energy, nor performed employee training and executed periodic inspections on the control of hazardous energy.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  Truss Manufacturing Plant on the North side near the ALS-276 CNC: On or about 5/17/2021, the employer exposed employees amputation hazards, in that the blade protective guard  on the Master Mechanic 10" Circular Saw was broken while employees cut wood.   b.  Truss Manufacturing Plant on the East side: On or about 5/17/2021, the employer exposed employees to caught-in and struck-by hazards created by rotating and moving parts, in that 2 bearing protective guards were missing and the right side limit switch (trigger) safety device was broken on the 2nd Alpine EasyRider Gantry while roof trusses were being assembled at the truss table.

29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  a.  Truss Manufacturing Plant and yard: On or about 5/17/2021,  the employer exposed employees to struck-by hazards, in that powered industrial truck (PIT) training was not provided to employees operating PITs such as, but not limited to the LULL CK-37, JCB 940 and Yale 60 VX while moving materials around the plant and yard.
