29 CFR  1926.704(d): Lifting hardware for precast concrete members was not capable of supporting at least five times the maximum intended load applied or transmitted to the lifting hardware.   a) 2682 Limekiln Pike, Glenside Pa, Worksite - On or about May 15, 2021 employees were engaged in activities involving the removal of concrete box beams from a bridge.  The tasks included installing engineer designed lifting hardware to the beams so that they could be hoisted away by a crane.  The hardware was not capable of supporting at least five times the maximum load applied or transmitted to the lifting hardware.

The crane operator set up beneath 38 kv energized power lines.  The crane operator did not determine if any part of the crane could get closer than the minimum approach distance of the power lines.  A planning meeting or site hazard assessment for the crane task was not conducted.
