29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. Employees were exposed to serious hazards. Employees engaged in tasks within the maintenance building were exposed to serious hazards in that combustible material, excess chemical waste, and industrial materials were being stored in the maintenance shop. Employees were exposed in that the maintenance shop was not kept in a clean, orderly and sanitary condition.

29 CFR 1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: Employees were exposed to explosion hazards. Employees engaged in activities within the maintenance building were exposed to burns and inhalation hazards in that the employer failed to properly store the compressed gas cylinder, containing LP-Gas, which was being stored in close proximity to flammable substances such as gasoline, oil and waste within the same maintenance building room.

29 CFR 1910.272(j)(2)(ii):The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators: Employees were exposed to health hazards. Employee's engaged in operations within the priority housekeeping areas located in the boot pit were exposed to health hazards in that the employer did not ensure dust was removed in the following instances: 1) Directly in front of the boot pit ladder, fugitive grain dust was measure approximately 1/2 inch deep 2) Directly in front of the elevator leg, fugitive grain dust was measured approximately 4 1/2 inches deep
