The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a) On or before 4/28/2021, the employer did not ensure path to ground was maintained when employees were spraying water around the testing tank to prevent employees from being shocked, the energized on/off switch for the testing tank is plugged behind the tank into an extension cord that is plugged into a wall 110v outlet.

Employees were exposed to a shock hazard due to the energized electrical cord located behind the testing tank being saturated with water while cleaning oil spills.

Employees were exposed to a shock hazard due to a flexible cord being used to run the electrical on/off switch being sprayed with water.

Safety signs, safety symbols, or accident prevention tags shall be used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145. a) On or about 4/28/2021 the employer did not ensure that safety signs were in place to warn employees of the electrical testing of the bucket truck liners. Employees were exposed to 100,000 volts of electricity during the testing of the bucket truck liners.
