29 CFR  1910.133(a)(3):  The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:   a. At the facility located at 3004 NE 21st Way in Gainesville, Florida: On or about May 18, 2021, the employer exposed employees to eye injuries from flying debris, in that the employer did not provide the appropriate eye protection for employees  who wore prescription lenses while engaged in drilling, sanding of wood cabinets and the use of various chemicals.

29 CFR  1910.213(c)(1): Circular hand-fed ripsaw(s) were not guarded by an automatically adjusting hood, which completely enclosed that portion of the saw above the table and above the material being cut:  a. At the facility located at 3004 NE 21st Way in Gainesville, Florida: On or about May 18, 2021, the employer exposed employees to laceration hazards caused by an unguarded saw blade and struck-by hazards caused by flying debris during the sawing process, in that the upper portion of the blade of a Delta brand, 10-inch table arbor saw did not have a guard.  b. At the facility located at 3004 NE 21st Way in Gainesville, Florida: On or about May 18, 2021, the employer exposed employees to laceration hazards caused by an unguarded saw blade and struck-by hazards caused by flying debris during the sawing process, in that the upper portion of the blade of a Saw Stop brand, table saw did not have a guard.

29 CFR  1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 P.S.I.:   a. At the facility located at 3004 NE 21st Way in Gainesville, Florida: On or about May 18, 2021, the employer exposed employees injury to the eyes and other body part injuries, such as, but not limited to respiratory system, in that  the compressed air was not reduced to less than 30 p.s.i., as employees used compressed air to clean machines, themselves and their clothing.
