On a 6:12 pitched roof of a temporary residential construction worksite located at 26 Woodside Drive in Palm Coast, Florida: On or about April 27, 2021, the employer exposed seven employees to fall hazards ranging from 9-foot to 15-foot, in that fall protection was not utilized by employees while performing roofing work.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. At the front roof eave of a temporary residential construction worksite located at 26 Woodside Drive in Palm Coast, Florida: On or about April 27, 2021, the employer exposed employees to a 9.6-foot fall hazard from a 6:12 pitched roof eave, in that the roof access ladder side rails were extended to 28-inches above the eave and did not provide the required 3-foot (36-inch) hand hold for the seven roofing employees who used that ladder.
