On or about May 11, 2021, an employee of American Asphalt of Wisconsin conducted welding operations inside the drum of a Bomag Type BW-284-AD Tandem Vibratory Roller while the environment contained flammable vapors resulting in the ignition of the vapors and a resulting flashback.

On or about May 11, 2021, an employee conducting arc welding on the drum of a Bomag Type BW-284-AD Tandem Vibratory Roller did not take precautions such as avoiding all possible sources of ignition and/or using local ventilation to prohibit the build up of vapor in low or confined areas when using Schaeffer's Citrol Multi-Purpose Degreaser and Talon Non-Chlorinated Brake Cleaner.
