29 CFR  1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o):  a) Shop area: On or about May 26, 2021, and at times prior, workers were exposed to amputation hazards where the top and bottom motor and belt drive of pumps did not have guarding to prevent access from the back of the motor and pump set.  b) Shop area: On or about May 14, 2021, workers were exposed to amputation hazards where the bottom motor and belt drive guard was not installed.  Note to a) above:  The belt and pulleys must be guarded from the back as well to prevent reaching in from behind or over/under the existing guard.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative:  a) On or about March 12, 2021, and at times prior, workers were exposed to amputation hazards when performing maintenance and replacement tasks on the vehicle conveyor chain while the motor energy was disconnected by an emergency stop button and was not locked out to prevent restart.  b) On or about March 12, 2021, and at times prior, workers were exposed to air-blast hazards when performing maintenance and replacement tasks on the vehicle conveyor chain while the air supply was in the off and depressurized state without being locked out.  c) On or about May 14, 2021, and at times prior, workers were exposed to amputation hazards during replacement of motor and pump drive belts while the motor was not locked out to prevent unexpected restart of the motor.  The Richie's Parker Express Car Wash, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(1), which was contained in OSHA inspection number 1501908, citation number 1, item number 2 and was affirmed as a final order on (date), with respect to a workplace located at 9996 South Twenty Mile Road, Parker, CO 80134.
