29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s),such as those from severe cuts or lacerations: On or about May 18, 2021, employees were removing/installing cutting heads on to wood lathing machines without the use of gloves. This condition exposed employees struck-by hazards.

29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle: On or about May 18, 2021, an employee removed the cutting head of a woodworking machine without implementing lockout/tagout procedures. This condition exposed employee to struck-by hazards.
