(a) Paradigm Construction, LLC at 4812 South Hosta Avenue in Sioux Falls, SD 57108: On or about and at times prior to May 18, 2021, the employer did not ensure employees that utilized personnel platforms attached to the forks of Gehl  RS8 telehandlers to access and egress from the roof of an apartment building were protected from fall hazards. The personnel platform was not equipped with an appropriate guardrail system and employees were not utilizing personal fall arrest systems on the platform. This condition exposed employees to fall hazards approximately between 10-20 feet.

This condition exposed employees to fall hazards of approximately 20 feet.   The Paradigm Construction, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1254806, citation number 1, item number 2 and was affirmed as a final order on 11/13/2017 , with respect to a workplace located at 5140 S. Greystone in Sioux Falls, SD 57108.  Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
