DBA Jack & Jill DSD - Forklift charging area: The facility did not protect the battery charging apparatuses from damage. The charging cables on a Exide-Yuasa battery charger model #W1-18-865, S/N ZB46791 had damaged cords with exposed copper conduit and the charger outer casing was damaged, observed on or about 05/19/2021.
