29 CFR  1910.157(d)(4):Portable fire extinguishers for use on Class B fires were not distributed so that the travel distance from the Class B hazard area to any extinguisher was 50 feet or less:  At the Outside Diesel Storage, employees engaged in fueling activities are exposed to fire, struck-by and explosion hazards in that the employer did not provide a Class B fire extinguisher within 50 feet.

29 CFR  1910.215(b)(9):The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  At Machine Maintenance Shop, employees engaged in maintenance activities at the Dayton Pedestal grinder in that the tongue guard was adjusted to 7/8 of a inch from the grinding wheel, exposing employees to struck by and caught in hazards.

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  At the TFLOS Printers Line, employees engaged in production activities were exposed to electrical shock and fire hazards in that the ground pin was missing on a electric pallet jack charger.
